Start Date/length of assignment: 10/16/2024- Outgoing  
Worksite: HCA North Florida Regional Medical Center
Schedule: FT Hours – Five 8-Hour Shifts per Week (7 AM Start Time); weekday call (3:30p-7a); weekend call (7a-7a)
On call: Weekday beeper 3:30p-7a; Weekend call: 7a-7a 24 hour (Preferred to take 1 call shift per month)
Cases: General, Ortho, GI-Endo, Eyes, Healthy Peds, Urology, Robotics, OB/GYN, Bariatrics,   Vascular, cardiac ablations, interventional radiology, neuro-interventional radiology, neuro,    bronchoscopy. No trauma and cardiac.
Supervision Ratio:  
ORs: 29 OR Rooms
How long does Credentialing Take: 90 days; Cross-Credentialed at North Florida Surgical Pavilion
EMR: Meditech
Special Requirements: Must have FL license, Must be Board Certified 

• Must not have any prior malpractice history or background issues

• Details: No Providers who worked for Envision within past 12 months and that live within 75 miles of the site 

• No Providers who worked for this site or an affiliated site in the past 24 months  
• No Providers that worked at this site or affiliated site that is within 75 miles of this site  
• New Candidates that have never worked for Envision must live outside the 75 mile radius of this site 
• All CRNA’s must have 2+ years of work experience as a CRNA
• No recent graduates

Additional Notes- **Please note that a call interview/Zoom Meeting with the Site Medical Director (SMD)/Clinical Leadership at the site will be required.
