Job Summary

Specialty: FP, IM, DO Geriatrics 
Start Date: September 16th or as soon as credentialed. (2 FTE’s) 
Contract Length: Until Perm provider is identified. 
Schedule: Monday – Friday 8a – 5p Call 1:3 Night call and 24 hour weekend call (can be done from home) 
Work Details: 
•    Support staff: Nurses, LCSW, Case workers 
•    Amount of coverage needed per month: 80 hours per week. 1 due to provider leaving and one due to growth 
•    Patients per day: 1-6/7 (can fluctuate depending on severity of patients illnesses/meds etc and how much time they need to spend with them 
•    Patient population/age: 55 and older able to live safely in the community.

Key Responsibilities
•    Performs comprehensive history and physical on new referrals.
•    Performs interval history and physicals on established participants completed quarterly and as indicated.
•    Evaluates and treats participants with episodic illnesses.
•    Refers participants to medical specialists as indicated.
•    Admits participants to the hospital.
•    Provides primary care (attending) responsibilities for management
•    Provides updates to the team on a regular basis.
•    Judiciously utilizes specialty consultants.
•    Responsible for coordinating discharge planning with the team.
•    Manages all care of participants in the nursing home:
•    Provides regular visits.
•    Performs telephone contacts with nursing home staff as required.
•    Admits nursing home participants to hospital when necessary.
•    Actively participates in family conferences.
•    Coordination of 24-hour care delivery.
•    Documents participant changes appropriately in the medical record and communicates participant change to team members.
•    Participates as a member of the Interdisciplinary Team. Takes part in participant care planning.
•    Attends staff meetings.

Practice Setting: 
•    Clinic/Private Practice 
Requirements: 
•    Board Certification: BC – will look at Non boarded but want reasoning for boards lapsing. 
•    License required: NC – due to time frame MUST be NC licensed.  valid NC driver’s license and vehicle. 
•    Certifications required:  Experience caring for elderly and in good standing and enrolled with Medicare and Medicaid. Need schools attended, NPI and work experience.

•    Proven clinical leadership; Ability to excel with others in a team environment; Act within the scope of his or her authority to practice; Meet a standardized set of competencies for the specific position description established by PACE of Guilford and Rockingham Counties and approved by CMS before working independently.
Credentialing Timeframe: 30 -45 days
